your Galaxy Certificate in pdf format for printing and for ROPA applications. KEEP THIS FILE!

When you are ready to submit your next Galaxy application;
Use the attached Excel File;
DO NOT remove any sheets or Change the Protected Sheets Tabs, they will be used by the SRD
Delete all data on the “New Acceptances” and “New Titles” sheets
Put New Acceptances and Titles on the “New Acceptances” and “New Titles” sheets
Email the Galaxy Application Excel File to the PIDC SRD Coordinator
Please follow instructions and samples in the latest version of the Applications Information & Instructions available on the PSA website.
